University College Plymouth St Mark & St John

The Photograph Identification Form is to be used where you are unable to attend the University College to present your identification documents (click here for a list of suitable documents) in person.  We must check your identity face-to-face in order to comply with Criminal Records Bureau regulations. 

You will need to attach a passport size photograph to the form then the form will need to be signed by a professional person ie a teacher etc to say that you are the person in the photograph.  The same person will also need to sign the photocopies to say that they have seen the original identification documents.

If you do not use the Photograph Identification Form, you must present your ID documents in person at UCP Marjon so that we can check your identity.

Please return the completed authorised form (with photograph), together with the authorised photocopies of the identification documents and the completed CRB application form with your payment of £54.00 (cheques payable to College of St Mark & St John) to:
