What do we do?

We provide a community based approach to working with people with long term health conditions.  Delivered in partnership with Livewell Southwest the programme is specifically designed to help participants with a diagnosed long term condition to develop knowledge, skills and confidence to take positive steps in improving their quality of life and manage their own health and wellbeing. 

How do we do it?

During each weekly session participants work in groups and individually with our skilled health coaches to develop personalised goals that promote a healthy lifestyle.  Adopting a strengths based approach participants learn how small changes in things such as physical activity, healthy eating, and sleep quality can lead to improvements in long term healthy living. 

We take an individual approach and focus on participants individual goals.  Our ethos is to find activities that are enjoyable and sustainable and to work on making healthy living part of participants daily routines.

Who is it for?

This programme is for adult participants recently diagnosed with a chronic or long term condition. We work closely with GP practices across Plymouth so individuals interested in joining the programme can do so by contacting their GP.

There are are weekly 2 hour sessions over six weeks. There are three groups running out of Four Greens Wellbeing Hub, Oakside Surgery and Honicknowle Community Centre and Ernesettle Primary Care Centre.
